Brain MRI

Structural brain imaging, marketed in some executive-health packages as an early screen for silent stroke, aneurysm or neurodegeneration. It can detect small vessel disease, silent infarcts and structural abnormalities, all of which matter a great deal when found in a patient who has symptoms. The screening question is whether finding them in someone who has none changes anything.

Why it grades Weak

In asymptomatic adults, screening brain MRI carries a well-documented rate of incidental findings — commonly cited in the 2–8% range on research brain MRI, depending on definition and age — the large majority of them benign or of uncertain significance. They generate follow-up scans and anxiety without a demonstrated pathway to improved outcomes for the average person screened. No major guideline body recommends brain MRI as a general population longevity or dementia-risk screen.

One of the more clearly overhyped items in premium "executive health" packages — dramatic-sounding, expensive, and without a validated screening indication in a healthy adult with no symptoms or focal risk factors.
