Testosterone test cost in Singapore — and why "optimisation" testing gets ahead of the evidence

Total testosterone is one of the most heavily marketed items in Singapore's executive-health and "male optimisation" packages — cheap to run, easy to explain, and attached to symptoms (energy, libido, muscle mass) that are common and non-specific enough that almost anyone can be persuaded a low number explains how they feel. The evidence supports testing men who have symptoms of hypogonadism, confirmed with a second sample if the first is low — not a routine annual draw for asymptomatic men chasing a marketing-defined "optimal" number.
What the evidence says
The TRAVERSE trial (Lincoff AM, Bhasin S, Flevaris P, et al., N Engl J Med 2023, PMID 37326322) randomised 5,246 men aged 45–80 with hypogonadism symptoms, a confirmed low testosterone level, and elevated cardiovascular risk to testosterone gel or placebo. Testosterone replacement was non-inferior to placebo for major adverse cardiac events — resolving a long-running safety question about testosterone therapy and heart risk. That's a real, hard-won piece of evidence, but note exactly who it applies to: symptomatic, confirmed-low men. Nonfatal arrhythmia, including atrial fibrillation, was more frequent in the testosterone arm, and acute kidney injury and pulmonary embolism were also numerically higher — real trade-offs, not a clean "all clear."

Public/subsidised polyclinic testing wasn't independently verified for this article (loose search estimates suggested roughly S$50–120 subsidised for Singaporeans/PRs and S$100–200 unsubsidised) — treat that as a rough contextual range only, not a cited figure. Testosterone testing is not Medisave- or CHAS-claimable in Singapore.
Who should prioritise it
Test if you have actual symptoms plausibly linked to low testosterone — reduced libido, erectile dysfunction, unexplained fatigue, loss of muscle mass, or a clinical condition associated with hypogonadism (e.g. pituitary disease, chronic opioid use, significant obesity) — and confirm any low result with a second morning, fasting sample before acting on it. Skip it as a routine "am I optimal" annual check if you're asymptomatic: a low-normal number in someone who feels fine isn't a validated reason to start therapy, and the marketing framing of testosterone as a general longevity lever gets ahead of what TRAVERSE and the guidelines actually support.
Bottom line
At S$30.60–38.15 for a single total testosterone draw, this is one of the cheaper tests on this site — which is exactly why it's easy to over-order. The evidence bar for testing isn't cost, it's symptoms: test a symptomatic man with a confirmed low level, don't test an asymptomatic one chasing a number. If a package sells testosterone testing as a standalone "optimisation" or wellness check outside that context, the TRAVERSE evidence and Endocrine Society guidance don't back that framing.
